Brain energy and mental fatigue:
what glucose, blood flow, creatine and NAD+ really do.
The brain requires a continuous supply of energy, but mental fatigue is not simply an empty fuel tank. Performance depends on glucose and oxygen delivery, cellular ATP regeneration, sleep, network efficiency, motivation and the demands of the task.
The brain needs constant energy,
but mental fatigue is not simple fuel depletion
Neurons use energy to maintain electrical gradients, release neurotransmitters, recycle signalling molecules and restore the cell after activity. The brain therefore depends on a continuous supply of fuel and oxygen rather than storing a large reserve for later use.
However, feeling mentally exhausted does not prove that the brain has “run out” of glucose or ATP. Mental fatigue is a broader psychobiological state shaped by prolonged cognitive demand, sleep loss, stress, boredom, motivation, task value and changes in neural signalling.
- Substrate availability: glucose is the main everyday fuel, while lactate and ketones can contribute under specific conditions.
- Oxygen delivery: oxidative metabolism depends on adequate circulation and oxygenation.
- Cellular conversion: mitochondria convert available substrates into ATP.
- Energy buffering: phosphocreatine helps regenerate ATP rapidly when local demand rises.
The sensation of mental effort reflects how demanding a task feels relative to current capacity. It is not a direct measurement of how much glucose or ATP remains in the brain.
Mental fatigue and cellular energy overlap, but they are not identicalGlucose is essential,

Under ordinary fed conditions, glucose is the brain’s principal fuel. This biological fact is sometimes converted into the simplistic claim that consuming sugar will reliably improve concentration or memory.
A systematic review identified 37 intervention trials examining glucose or sucrose and cognition in healthy people. Results were mixed. The meta-analysis found a benefit only for immediate word recall in a subset of parallel-design studies, and many included studies carried a high risk of bias.
Glucose Did Not Produce a Universal Cognitive Benefit
The pooled evidence showed a significant benefit only for one immediate verbal-memory outcome in a subset of studies. The authors concluded that higher-quality, standardised trials were needed. PMID 32585003
- Avoid the “brain needs sugar” shortcut: the brain needs glucose, but the body regulates and supplies it from meals and stored fuel.
- Prioritise meal quality: protein, fibre and minimally processed carbohydrate generally support steadier satiety and metabolic health.
- Do not self-treat symptoms: recurrent shakiness, confusion, sweating or suspected low glucose deserves proper assessment.
- Consider the whole pattern: sleep, hydration, caffeine timing, medication and workload often explain “afternoon brain fog” more convincingly than sugar deficiency.
Circulation supports the brain,
but more blood flow is not automatically more cognition
Cerebral blood flow supplies oxygen and substrates while helping remove carbon dioxide and other metabolic products. Blood flow adjusts regionally as networks become more active, which is one reason brain-imaging methods can infer changes in neural activity.
Exercise can improve cardiovascular fitness and cerebrovascular function. A systematic review of 45 human intervention studies found relatively consistent increases in blood flow within areas such as the anterior cingulate cortex and hippocampus in MRI studies, while whole-brain and ultrasound findings were more variable.
Exercise Changed Some Measures of Cerebral Blood Flow
Regional increases were observed more consistently than whole-brain changes. Relationships between improved fitness, regional blood flow and cognition were present in some studies but were not uniform. PMID 36170974
Exercise Improved Global Cognition and Executive Function
Exercise interventions improved global cognition and executive function in adults with subjective or objective cognitive decline. Cerebral blood flow improved in one longer trial, but the change was not directly linked to cognitive improvement. PMID 37260849

Blood pressure, smoking, glucose control and physical activity affect long-term brain health.

Short periods of activity can improve alertness and reduce prolonged sedentary time.

Prolonged effort can change
perception, accuracy and decision-making
Mental fatigue commonly follows sustained attention, complex decision-making, prolonged screen work or repeated inhibition of distractions. The subjective experience can include effort, irritability, lower motivation and reduced willingness to continue.
Performance effects depend on the domain. A 2026 systematic review in sport-related settings found that experimentally induced mental fatigue was associated with poorer reaction-time, technical and decision-making performance. The evidence was most consistent for decision-making, while reaction-time evidence was based on fewer studies.
Mental Fatigue Was Associated with Poorer Decisions and Technical Performance
The review separated reaction time, technical execution and decision-making rather than combining them into one performance score. Decision-making showed the most consistent adverse effect. PMID 42216083
- Define one outcome: clarity reduces the working-memory load created by vague tasks.
- Batch interruptions: check messages at planned intervals instead of switching continuously.
- Use genuine pauses: a brief walk or change of environment is different from replacing one screen with another.
- Match work to alertness: complete complex, safety-critical or creative tasks during your strongest period where possible.
Creatine has a credible energy role,
but the cognitive evidence remains domain-specific
Creatine and phosphocreatine form a rapid energy-buffering system that helps regenerate ATP. This system is best known in skeletal muscle, but it also operates in the brain.
A 2024 meta-analysis included 16 randomised controlled trials involving 492 participants. Creatine improved memory and time-based measures of attention and processing speed, but did not significantly improve overall cognition or executive function.
Some Cognitive Domains Improved, but Global Cognition Did Not
Benefits were reported for memory, attention time and processing-speed time. Evidence did not show significant improvement in overall cognitive function or executive function. PMID 39070254
Brain Creatine Increased, While Cognitive Findings Were Equivocal
An independent review concluded that supplementation can increase brain creatine, but cognitive findings remained inconsistent. Better trials are needed in sleep-deprived, stressed or lower-creatine populations. PMID 38582412
Raising blood NAD+ is achievable,
but cognitive improvement is not yet established
NAD+ participates in redox reactions, cellular energy metabolism and enzyme systems involved in repair and signalling. Nicotinamide riboside, or NR, is an NAD+ precursor that reliably raises NAD+-related metabolites in human blood.
That biochemical effect should not be confused with proven cognitive enhancement. In a 2026 double-blind pilot trial involving adults with amnestic mild cognitive impairment, NR doubled blood NAD+ but did not improve cognition, total cerebral blood flow or blood pressure over 12 weeks.
NR Raised NAD+ but Did Not Improve Cognition
The intervention was well tolerated and doubled blood NAD+ in the NR group. No improvements were found in the primary cognitive outcome or total cerebral blood flow. Exploratory regional findings require confirmation. PMID 42478598
A 2.6-Fold Blood NAD+ Increase Did Not Alter Cognition
NR was well tolerated and substantially increased blood NAD+, while cognitive and psychometric measures remained stable. The study was designed primarily to assess safety. PMID 37994989
A supplement can produce a measurable biochemical change without producing a meaningful improvement in how someone thinks, remembers or performs.
