Breathing, HRV and the autonomic nervous system:
what actually helps stress recovery.

Slow breathing can alter the timing between heartbeats and increase breathing-related heart-rate variability. That can be useful for short-term regulation and biofeedback, but HRV is not a direct diagnosis of stress, anxiety, burnout or “vagal tone”. Measurement quality, breathing rate, posture, recent exercise, sleep and individual baseline all affect the result.

Breathing can influence heart rhythm,
but HRV is not a complete stress test

Heart-rate variability, or HRV, describes variation in the time interval between consecutive heartbeats. Those intervals change because the heart is continually responding to breathing, blood-pressure regulation, posture, movement, temperature, hormones and signals from the autonomic nervous system.

Slow breathing can produce a clear short-term rise in breathing-related HRV. This does not mean the person has instantly “healed the nervous system”, switched off the sympathetic system or eliminated the source of stress. It means respiration and cardiovascular control have become more rhythmically coupled during that period.

Four Important Distinctions
  • HRV is a physiological signal: it reflects changing heartbeat intervals, not a direct measurement of thoughts or emotions.
  • Breathing changes HRV: comparing readings taken with different breathing patterns can be misleading.
  • Higher is not always better: interpretation depends on age, rhythm, health, activity and the measurement used.
  • Stress is multisystem: sleep, behaviour, appraisal, hormones and environment cannot be reduced to one wearable number.

A useful HRV reading can add information about cardiac autonomic modulation. It cannot tell you, by itself, whether you are psychologically resilient, clinically anxious or properly recovered.

Sympathetic and parasympathetic activity
are not a simple on-off switch

The autonomic nervous system helps regulate heart rate, blood vessels, digestion, temperature and other functions that usually operate without conscious control.

Sympathetic activity supports mobilisation during exercise, standing, heat, excitement and threat. Parasympathetic cardiac activity contributes to beat-to-beat slowing and recovery. Both branches can be active at the same time, and neither is inherently good or bad.

Recent methodological guidance warns against treating HRV as a direct measure of cardiac sympathetic activity or a simple sympathovagal balance. Breathing, recording method, duration, age, exercise and environmental conditions all affect interpretation. PMID 42495990

Why Breathing Changes HRV

The heartbeat normally speeds and slows
across the respiratory cycle

Heart rate commonly rises slightly during inhalation and falls during exhalation. This breathing-related pattern is called respiratory sinus arrhythmia. It is a normal physiological rhythm rather than a dangerous heart arrhythmia.

Slower breathing can amplify the oscillation because respiratory timing interacts with blood-pressure reflexes and cardiac vagal modulation. The exact response depends on breathing frequency, depth, posture and the individual.

Voluntary Slow Breathing Increased Vagal-Mediated HRV Measures

The review found increases during slow breathing, immediately after a single session and after multi-session interventions. The authors described slow breathing as a low-cost adjunct with relatively few expected adverse effects. PMID 35623448

Respiration Is a Major Source of HRV and Must Be Included in Interpretation

A recent review emphasised that respiratory modulation has a defined physiological origin and can be confused with broad claims about autonomic state when breathing is not measured or controlled. PMID 42162693

If breathing rate changes between two readings, the difference in HRV may partly reflect the measurement conditions rather than a meaningful change in recovery.

Breathing is not noise in HRV data. It is part of the signal.
A Practical Breathing Exercise

Gentle, regular breathing is safer
than forcing the largest possible breath

The NHS calming exercise advises allowing the breath to move into the belly as comfortably as possible, without forcing it. Breathe gently and regularly, counting up to five on the way in and again on the way out if comfortable, and continue for at least five minutes.

  • Choose a supported position: sit, stand or lie somewhere comfortable.
  • Relax unnecessary tension: especially around the jaw, shoulders and abdomen.
  • Breathe gently: let the breath move lower without deliberately gulping air.
  • Use an even count: count one to five in and one to five out only if comfortable.
  • Practise regularly: skill develops more reliably outside the peak of a stressful event.

Avoid Over-Breathing

Deeper is not always calmer,
especially when breathing is already too large or fast

Stress and panic can produce rapid or excessive breathing. Over-breathing lowers carbon dioxide and may contribute to dizziness, tingling, chest tightness, breathlessness and feelings of unreality.

Trying to take repeated maximal breaths can worsen those symptoms. The aim is gentle, controlled breathing rather than pulling in as much air as possible.

Pause and Seek Advice If
  • The exercise increases dizziness or tingling: return to normal breathing and rest.
  • Breathlessness is new, severe or unexplained: do not assume it is stress.
  • There is chest pain, fainting or blue discolouration: urgent medical assessment may be required.
  • Panic symptoms are frequent or disabling: breathing practice can support treatment but should not replace it.

HRV Biofeedback

Biofeedback turns the physiological signal
into a trainable skill

HRV biofeedback displays heartbeat timing while the user practises paced breathing. Many protocols identify a breathing rate that produces a strong and stable oscillation for that person.

A 2017 meta-analysis included 24 studies and 484 participants and found a large average reduction in self-reported stress and anxiety. The authors also noted the need for more well-controlled studies.

How to Measure HRV More Consistently

Compare similar conditions
rather than reacting to every daily fluctuation

HRV changes with age, fitness, recent exercise, alcohol, illness, food, hydration, sleep, temperature, time of day, posture, breathing and recording duration. A meaningful personal trend requires reasonably consistent conditions.

A Better Morning Measurement
  • Use the same device and metric: different platforms may calculate and filter data differently.
  • Measure at a similar time: ideally before caffeine, food and training.
  • Use the same posture: lying, sitting and standing produce different cardiovascular demands.
  • Do not deliberately change breathing: unless the purpose is a paced-breathing test.
  • Interpret a trend: one unusually low or high value is less informative than repeated readings alongside symptoms and workload.

The review identified age, physical activity, alcohol, smoking, medicines, diet, time of day, temperature, noise, posture, recording duration and respiration as important influences. PMID 39351472

Wearables and Stress Scores

A wearable can detect a pattern,
but it cannot know the meaning

Wrist devices often estimate pulse timing using photoplethysmography rather than recording the heart’s electrical signal directly. At rest, some wearable measurements correlate well with ECG-derived intervals. Accuracy often falls during movement and exercise.

Proprietary “stress” and “readiness” scores combine signals using algorithms that may not be fully disclosed. A low HRV reading after hard training, poor sleep or illness does not necessarily mean psychological stress. A normal score does not rule out anxiety, burnout or medical illness.

  • Minute 9 to 10: decide whether you need movement, food, rest, conversation or professional support.

Breathing is most useful when it creates enough physiological and mental space to choose a better next action. It cannot compensate for unsafe workload, severe sleep restriction, unresolved conflict or a medical problem.

When Professional Support Matters

Breathing exercises are supportive,
not a substitute for assessment or therapy

Seek medical advice for new or unexplained palpitations, fainting, chest pain, significant breathlessness or a persistently irregular heartbeat. Speak with a GP or use NHS Talking Therapies when stress, anxiety or panic is substantially affecting daily life.

Do Not Rely on a Wearable Alone If
  • You have new cardiac symptoms or an alert suggesting an irregular rhythm.
  • Anxiety or panic is frequent and causing avoidance or loss of function.
  • You are repeatedly checking the score and the monitoring itself is increasing distress.
  • You feel unsafe or unable to cope and need urgent mental-health support.
